A person fills in the form at https://joinopenfile.com/trademark/start and signs. It covers one mark, and the price is $200, attorney included, plus the USPTO's $350 per class.

The form asks for the mark, what you sell, the owner, and whether you already sell under the name. It checks the name against USPTO records as you go.
